Hacienda HealthCare reports another patient injury

PHOENIX — The Arizona Department of Health Services has confirmed that Hacienda HealthCare has self-reported an injury involving a patient to state authorities, although officials say they cannot comment on the on-going investigation.

The response came after ABC15 reached out to the health department after getting tips involving a patient who suffered a broken arm.

Multiple sources tell ABC15 the incident took place last month and that Hacienda administrators took disciplinary action against some staff members because of this incident.

ABC15 reached out to Hacienda officials to get more information but were told they could not comment due to medical privacy laws.

This investigation comes while Hacienda is already under the microscope for citations involving another patient in June 2019. Health survey reports state staff found between 6-12 maggots in a feeding tube or tracheostomy wound of another patient.

State officials say they are still working with Hacienda staff on corrective measures to address that issue.

Earlier this year the Centers for Medicare and Medicaid Services (CMS) also informed Hacienda they were terminating their Medicaid certification.

The healthcare facility is appealing that decision. ABC15 has reached out to CMS to find out what the status of this case is and are still waiting to hear back.
