According to the Arizona Auditor General's Office, required inspections of child care providers were not always completed in past years.
The newly released audit suggests the Arizona Department of Economic Security, which is in charge of child care services, failed to perform some on-site inspections between 2012 and 2014.
The audit also found that inspectors did not always assess provider compliance with child care requirements and had "not established procedures to effectively receive, investigate and monitor complaints".
