PHOENIX — On the first Tuesday of every month, ABC15 takes action to share restaurant report cards from around the Valley. In October, 18 restaurants or food establishments failed to receive a "C" or better on their health inspections.

PHOENIX
Chevron
4352 E. McDowell Rd., Phoenix AZ 85008
Grade: D; see inspection report, here.
Violations: Spatula and baking sheets for cookies being stored as clean contained excessive amounts of food residue. Dishware was properly cleaned during inspection; Iced coffee machine had excessive build-up of food debris on nozzle during inspection. Machine was posted as "Out in service" during inspection and will be properly cleaned prior to re-use; Containers of open/used pesticides (Hot Shot) were found on the shelf across from 3-compartment sink. Do not administer pesticides at establishment; Excessive amounts of drain flies observed in and around cabinets underneath soda fountain and inside pastry display case.
Los Taquitos

Violations: A kitchen employee was observed washing their hands for a total time of 4 seconds. PIC was advised to train all employees to properly wash their hands for a total time of 20 seconds; Employee was observed cleaning then putting on gloves to work with food without washing their hands. PIC was educated that hands need to be properly washed between switching tasks and between glove changes; Dicer was found to be stored as clean but when inspected dried food debris was found in the rubber pieces of the dicer. When the dicer was disassembled there was organic matter found between the parts of the dicer. PIC was advised to inform cleaning staff to disassemble the parts before every cleaning.
La Marquesa

Violations: The establishment has Jericalla, a custard-like dessert. The PIC stated that the desert is not made in the establishment, and is instead brought in by an unpermitted vendor that makes the dessert in their private residence; a carton of raw shelled eggs were stored directly on top of a mesh bag of ready to eat onions. All raw animal proteins MUST be stored beneath ready to eat food items to prevent cross contamination. The PIC moved the eggs to prevent cross contamination; Observed several bags of cooked goat meat dated 9/23. A container of cooked vegetables (including carrots) and a container of cooked beans were dated 9/20. All perishable food items MUST be discarded within 7 days of being cooked/prepared, including the initial day of preparation. The foods were embargoed.
Giuseppe's Italian Kitchen

Violations:Employees were observed washing their hands at the prep sink and not the designated handwashing sink. Employees were advised that the only place to wash their hands is the handwashing sink; A slicer was observed to have food debris on the blades at time of inspection. An employee stated that the machine was not used that day. Employees were educated that the slicer needs to be properly washed after every use and the slicer was disassembled and properly washed at time of inspection; A container of cooked beef and onions was in the cold holding unit but was dated 10/3, which was supposed to be thrown away on 10/9 - the day before inspection. Employees were educated on proper date marking procedures and to add 6 days to the day the food is cooked.
Chevron/4 Sons Food Store

Violations:Violation documented due to no certified food service manager certification available, priority violations documented on this report and person in charge unable to demonstrate applicable food safety knowledge at time of inspection; No paper towels at pizza area handsink, provided at inspection. Second consecutive violation; Three open containers of hot dogs do not have any date marking. As per employee on last visit they usually sell a bag of hot dogs in 2-3 days from opening. Person in charge unsure when hot dogs were opened as per previous employee they sell each bag once opened within 2-3 days they sell.
Sunrise Catering (mobile food unit)

Violations: Observed raw shelled eggs stored above RTE food such as deli meats and cheese. Also observed raw pork chorizo stored above hot dogs and pre-cooked chicken; Observed the base of the faucet at the three compartment sink corroded and in need of replacement. Ensure that the faucet is replaced so that base is properly sealed prior to next routine inspection; Mobile Food Type III is not operating as classified. Observed multiple TCS items that were to be carried over from the days work to tomorrow per PIC. TCS food included: chicken patties, pork chorizo, ham, pastrami, raw chicken, raw beef, roast beef, and eggs. Food was embargoed at the time of inspection. Ensure that TCS items are disposed of at the end of operation.
Carniceria Mi Ranchito
5053 W. Southern Avenue, Laveen AZ 85339
Grade: D; see inspection report, here (meat market) and here (restaurant).
Violations: Chef went to wash hands and simply rinsed hands with water. Had PIC explain proper hand washing to employees. Please ensure employees are scrubbing hands with soap for at least 15 seconds before rinsing; Chef was cleaning cutting boards with a sanitizer rag and then changed gloves and began working with food without performing a hand wash in between. Explained to PIC that a hand wash should occur every time a task change occurs. PIC explained this to employees; Two live cockroaches were observed in the kitchen. PIC was able to provide evidence of pest control and the document demonstrated that the company had sprayed that morning. A re-inspection will occur in ten days to ensure the issue has been taken care of.
Carniceria Agave
3525 W. Southern Avenue, Phoenix AZ 85041

Violations: A chef in the kitchen was handling labels and money, changed gloves and changed task without a hand wash in between. Spoke with PIC and explained that a hand wash should occur every time a change of task occurs. Another employee did the same and wiped her hands on her apron, and then put new gloves on before working with food; Two large tubs of salsa in the walk-in were well out of date. Salsa rojas had been made 10/19 and salsa habanero was made 10/17. Had PIC discard these at time of inspection. Explained proper disposition timeframe to PIC.
EAST VALLEY
Kyoto Bowl

Violations: Heads of lettuce and box of green peppers were molded. Food was embargoed; Walk in cooler was holding cooked chicken, cut cabbage, tofu, egg rolls, cream cheese, cooked crab, sweet egg, stir fry noodles (labeled as keep refrigerated), salmon, and sushi roll at 47F-48F. Food was embargoed; Back storage area and coolers that are not in use are full of chairs and other types of equipment. Remove.
Port of Subs
1435 E. Warner Road, Gilbert AZ 85296

Violations: Observed employees wash hands for only 5-7 seconds. Had employee wash hands correctly. Discussed proper handwashing procedure with person in charge; Observed employee run the register and handle money, then proceeded to don gloves to work with open foods. Discussed "when to wash" with person in charge at time of inspection. The hands may become contaminated when the food employee engages in specific activities; Observed employee use the prep sink to wash hands. discontinue this practice. It is important that handwashing be done only at a properly equipped handwashing facility in order to help ensure that food employees effectively clean their hands.
Filiberto’s Fresh Mexican Food

Violations: Observed shredded cheese (47 *F) and shredded lettuce (47 *F) inside walk-in cooler. Cheese had been prepared the night before and was observed packed, inside 6 inch plastic container with a lid. Lettuce was prepared at 8 am in the morning per employee and had been packed in deep plastic buckets with lids. Person in charge discarded at the time of inspection; Observed rice holding between 126-137 *F inside hot holding unit. Container was not fully submerged in water. Rice was reheated in microwave then placed back in unit in a smaller container so entire container was in contact with hot water.
Ahipoki Bowl

Violations: Observed cut leafy greens at prep table with internal temps o 57F. Leafy greens were discarded at time of inspection. Discussed proper cold holding temps with person in charge; Observed cooked rice being held as time without any time stamps. Person in charge had timed stamped the rice at time of inspection. Discussed importance of time stamping potentially hazardous foods; Observed toxic chemicals stored near handwashing sink by registered stored on a rack above napkins for customers. Chemicals were relocated at time of inspection.
America Marketplace
6802 E. Thomas Road, Scottsdale AZ 85251

Violations: Observed an open can of soda in the kitchen. The drink was removed from the kitchen. Discussed with the person in charge about how all drinks in the kitchen must have a lid and straw; Observed an employee dry their hands off with their apron and continue to put new gloves on. The person in charge had the employee wash their hands again and dry them with the paper towels at the hand sink; off their gloves and put new gloves on and continue to work on the line without the benefit of washing their hands. The person in charge had them wash their hands and put new gloves on; Observed a large accumulation of grease and dust built up in the filters to the vent hood system in the kitchen. Increase the cleaning frequency necessary to preclude any build up.
Firerock Country Club

Violations: On cook line, employee handled raw chicken with gloved hands, then continued handled cooking utensils without washing hands. Cooking utensil was moved to warewashing area and instructed employee to remove gloves, wash hands, and don new gloves to continue with task; At bar, employee cutting ready to eat fruit with bare hands. Per manager, fruit is for customer drinks. Cut fruit was discarded. Instructed employee to wash hands, don gloves to continue with task. Discussed proper glove usage and handwashing with manager and employees.
Fry’s Food Store

Violations: In back prep area, employee wearing gloves, adjusted pants and shirt and touched dirty apron, then continued with task of handling equipment without first removing gloves and washing hands after contamination had occurred. Instructed employee to remove gloves, wash hands, and don new gloves to continue with task; At front service deli, all slicers with accumulation of food debris on food and nonfood contact surfaces. Per log book, slicers had not been taken apart and properly warewashed >5 hours. Instructed employee to dismantle slicers and properly warewash; At hot holding front service case, fried and baked chicken past the 4 hour discard time. Employee was told twice to discard fried and baked chicken. Fried and baked chicken were discarded. See embargo form.
Yogi’s Grill and Sushi

Violations: Observed a male staff put new gloves on without washing his hands, when asked to wash hands, staff just run his hands under water then dried them off with paper towels. Requested staff to rewash hands follow the guide posted on the wall at the hand washing sinks. He did; Inside the line Refrigerator noted with excessive old food debris, spillage of food but never got to clean out so it is now dried out and coated on the surface of the inside of this unit and on the shelf inside. Requested to have this unit thoroughly clean and sanitize properly by re-inspection; On the stove, noted 2 pots of boiling water, the lids to these pots feel by touch as they were excessively coated with grease and organic matter. These are food related equipment and must be maintain cleaned and sanitary daily.
WEST VALLEY
Stingers Sports Bar

Violations: Due to violations noted on this report and employees unable to answer basic food safety questions, as well as no manager certificate present at restaurant, this violation is being written. Obtain manager certification and train all employee properly with regard to food safety as it pertains to your operation; Hand sink at bar and in kitchen both lacked soap and paper towels. Items were supplied during inspection; Hand sink in kitchen is blocked by a mop bucket and is tucked in a tough to get to corner by ice machine. Hand sink also has a sign that states "do not use." Spoke with owner on phone to expedite repairs. Sink can be used but leaks excessively; Observed excessive food debris and old food grime buildup on surfaces of slicer used to cut ham (only deli meat present). Employee began cleaning during inspection. Also excessive food and debris buildup on, around, and under cutting board on make table. Clean items more frequently

Violations: Staff touching RTE chicken wings and arranging them on plate with bare hands. Discussed no bare-hand contact parameters with staff and manager. Instructed manager to discard food or re-cook food. Manager will have staff re-cook food at time of inspection; Leaf wrapped rice with cooked meat wrapped in plastic string wrap inside of freezer and walk-in cooler. No package labeling on food item. Per manager, food is not prepared in establishment or at one of their other restaurants but is prepared in warehouse. Per manager, this food item is the only food prepared in warehouse and delivered to establishment. Manager is unsure if warehouse has a permit for food processing. Manager is unable to find invoice. Spoke to additional manager on phone and food item was purchased at local Asian marketplace; Internal temperature of 2 containers of water with rice scoops stored inside of container measured at 75 Deg. F. Instructed manager that in-use utensils can be stored in water above 135 Deg. F. or in a clean, dry container if wash, rinse and sanitized every 4 hours.
